Connie Cook

Connie Sue Cook, 71, Bartlesville, passed away Saturday evening, January 16, 2021 at Jane Phillips Medical Center.

Connie was born March 27, 1949 at Bartlesville to Joseph Charlie Slape and Virginia (Lea) Slape.  She attended school at Wann, Oklahoma and finished her education at Bartlesville.  Connie was a Certified Nurses Aide and spent most of her career working at Forrest Manor Nursing Center, Dewey, Oklahoma.  She was of the Baptist faith and was an avid Bingo player.  Connie enjoyed spending time camping and with her kids and grandkids.

Survivors include her mother, Virginia of Bartlesville; two children, Terry Cook and his wife, Donna and Melissa Jacobs and fiancée, Bobby Wahnee, all of Bartlesville; two brothers, Joseph Slape and his wife, Virginia of Dewey and Larry Slape and Esther Reed of Bartlesville; one sister, Diana Johnson of Tulsa, Oklahoma; 16 grandchildren and 36 great-grandchildren.  She was preceded in death by her father, Joseph Slape; by two daughters, Sherry Hockett and Pamela Eudy; by two brothers, Ernie and Ronnie Slape and by four grandchildren, Tommy Cook, Justin Jacobs and Jacob and John Eudy.
